FQHC Survey

In a joint effort to improve the relations between free clinics and federally qualified health centers, Free Clinics of Michigan will be collecting information via surveys from its member clinics to share with the Michigan Primary Care Association.  To do this effectively, a preliminary survey asks about the current state of relationships between free clinics and local health centers.  Then, for a period of six weeks, a second survey form will be open for free clinic to document individual interactions (positive or negative) experienced with local FQHCs.  Record each event individually by filling out the second form as many additional times as necessary.  This is an opportunity to better serve the patient population that seeks service from both sources.  Please help us build partnerships towards a common goal.
