Are you experiencing a rise in requests or need for this service? 2. Do you provide dental care? If not, where do you direct your patients? 3. Has the number of patients leveled off? Stayed the same? Increased? 4. Do you have a sustainability plan or future plans for the clinic? 5. Are you collaborating wit neighboring clinics, FQHCs, and/or health systems to coordinate and provide care for your patients that you cannot provide?
